The Survey question # 6 simply asks the nurses who opted out of performing Break Relief assignments if they will put their names on the Opt-out list themselves or if they want the Union to it for them.

If you are not willing to perform in a Break Relief RN role, please place your name on the list of nurses who opted out of being assigned to perform in a role of Break Relief Nurse. After the aggregate data of this Confidential survey is published, the “Opt Out” list will be posted on the CNA’s Bulletin Board located in our Break Room. Please indicate below if you will place your name on the “Opt Out” list yourself, or if you want the Union to do it for you.
